What You Need
A copy of the child’s certified birth certificate
A copy of the child’s immunization records
If you are newly enrolling in a North Carolina public school, please have a physician complete this North Carolina Health Assessment Form
Two forms of documentation verifying the names and addresses of parents/guardians (examples include driver’s licenses, identification cards with pictures, rental agreements, utility bills, phone bills)
Custody papers, if applicable

Chatham Early College: August 7, 2024
Traditional Schools: August 26, 2024
North Carolina legislation requires that a child must be age 5 by August 31 of the current school year to enroll in Kindergarten.
